Prescription Assistance Program
Assisting Uninsured/Low Income Patients With Affordable Medications
The Prescription Assistance Program is committed to providing assistance to patients who are experiencing financial hardship with the inability to afford long-term medications. The Prescription Assistance Coordinator works diligently with the Health Care Team to ensure that eligible patients are enrolled in the program.
This service is free and available for adults and children:
- Assistance for eligible uninsured/low income and elderly patients
- Proof of income required
- Assistance with applying to pharmaceutical companies to receive free or reduced price medications
- Assistance with long-term medications for health issues such as Diabetes, Asthma, Hypertension and other health issues
- Pharmacy discount cards available
- Assistance with locating other resources to provide assistance with short-term medications
- Provider referrals and self referrals are accepted
- Must be an established patient at one of the clinic locations

- Visit with your health care provider; let your provider know that you are having financial difficulty paying for your long term medication(s).
- Ask for a referral to the program or call the Prescription Assistance Coordinator at 806.743.9355.
- Call the Prescription Assistance Coordinator; you will be asked several questions to determine your need for Prescription Assistance.
- 4 to 6 weeks once you have completed/submitted the necessary application(s) to the Prescription Assistance Coordinator.
- The application(s) will be forwarded to your provider for his/her signature and written prescriptions. Once this process has been completed the Prescription Assistance Coordinator will mail/fax the entire application(s) to the appropriate Pharmaceutical company.
- If you meet the Pharmaceutical company eligibility guidelines, you can expect to receive your medication in approximately 4 to 6 weeks.
- At your Provider's office, the Pharmaceutical company sends your medication(s) directly to the provider's office, some send them to your home or allow you to pick them up at the pharmacy; depends on the medication.
- The Prescription Assistance Coordinator will send you a letter in the mail notifying you that your medication(s) have been received.
- The letter will indicate the dates and times that you may pickup your medication.
- One year as long as you are eligible, application renewal is due every year depending upon your financial situation and prescription coverage.
